The MedBen Worksite Wellness program is a sensible approach to employee health that combines traditional health services with a focused disease management program. Intensive service is provided to those with key chronic conditions while general health information is available to all employees.

MedBen Worksite Wellness is concentrated on those individuals with health conditions that typically cost the most to your health plan: diabetes, asthma and cardiac-related issues. Personal care is extended to participants by our wellness partner, Clinix, a URAC-accredited provider.

Getting Started
To determine the overall health status of the organization, MedBen will conduct a Company Health Risk Assessment (HRA). This assessment is a comprehensive analysis of historical medical claims data, along with other clinical data, like pharmacy claims, utilization review and case management reports. Program nurses then conduct an Individual HRA of program participants, beginning with those identified as having claims related to a key chronic condition – asthma, diabetes and/or a cardiac condition. Through this confidential one-to-one phone interview, nurses can uncover family history, lifestyle practices and other factors that also weigh into a person’s risk for certain conditions.

Employees who opt into the program will receive a Personal Care Plan. The assigned nurse will work with the participant to develop the care plan in accordance with his or her personal health goals and the clinical recommendations for existing health conditions. The plan incorporates the health information received via the Company and Individual HRAs. Through this highly interactive approach, health professionals are able to tailor the plan of care to an employee’s unique health situation.

Those who wish to participate for improved health, but who do not currently show signs of key chronic conditions, will be placed on a general health regime of activity.

Close Monitoring
Ongoing nurse coaching is provided through regular outbound phone calls by the assigned registered nurse.  A mutually agreed upon call schedule is established so that the participant and nurse can have ongoing meaningful dialogue regarding an individual’s progress toward established health goals. The nurse reinforces these verbal exchanges with health information sent via regular mail or e-mail. Each participant displaying chronic conditions will receive a year’s subscription to a health magazine that is focused on the management of that particular condition. 

Nurses coordinate their plans with the primary care physician when the situation warrants. In this way, the program is intended to augment the physician’s health recommendations. Regular follow-ups by the nurse with the program participant helps ensure that prescribed medications are being properly taken, tests are being conducted as suggested and physician checkups are being done.

Local Connections and Useful Reports
A unique feature of the program is that it can be wrapped around locally based wellness programs offered through a community hospital, YMCA chapter, or health service agency. These programs can be set up as a resource for the nurses to refer program participants to so that they can benefit from exercise and other wellness interventions as needed. 

Monthly and quarterly reports help the employer to see how well individuals are doing in the program and to see measured improvement in the overall health status of the workforce.
